Case Discussion

Characteristic features of empyema include loculated pleural fluid collection and (faint) split pleura sign. The patient had not undergone thoracentesis, so the air within the empyema is likely due to bronchopleural fistula formation. However, the actual fistulous connections are often difficult to see.
