Diagnosis almost certain

Presentation

Left orbital pain and mild proptosis.

Patient Data

Age: 55 years
Gender: Male
mri

Dural thickening and enhancement along the anterior aspect of the left middle cranial fossa -sphenoid wing- as well as at the lateral wall of the left orbit cavity -hyperostosis-.

The globe shows normal size and normal ocular contents. The lens is intact.

The rest of the ocular walls are smooth, sharply defined, and of normal thickness.

The optic nerve shows normal course and caliber.

The rest of the eye muscles are normally positioned and display normal width and course.

The retro-bulbar fat is unremarkable.

Age related brain parenchymal involution.

Case Discussion

En plaque meningiomas are characterized by diffuse dural involvement, usually associated with extracranial extension -in this case to the left orbital cavity-.

Hyperostosis is an important finding and critical to the diagnosis.
