Enchondroma with pathological fracture

Presentation

Trivial trauma followed by pain.

Patient Data

Age: 30 years
Gender: Female

An expansile lytic lesion with a narrow zone of transition, endosteal scalloping, and chondroid matrix is seen involving the metadiasphyseal region of the 4th metacarpal.

Associated cortical breach and pathological fracture are seen.

No periosteal reaction is seen.

No periosteal reaction is seen.

An expansile lesion with endosteal scalloping is seen in the 4th metacarpal. Matrix of the lesion shows calcification. No soft tissue is seen growing outside the bone. Pathological fracture is present.

On post-contrast images there is enhancement is seen along the scalloped margins.

Case Discussion

Findings that favor the diagnosis of enchondroma over low-grade chondrosarcoma are: 

  • young age

  • involvement of small bones (metacarpal)

  • the small size of the lesion

  • endosteal scalloping involves less than 2/3 of cortical thickness

  • absence of permeative bone destruction

  • soft tissue does not extend beyond the bone
