End-stage rheumatoid arthritis of the shoulder

Presentation

3-month history of left shoulder pain and swelling, with history of rheumatoid arthritis. Physical examination showed limited and painful active and passive range of motion of the shoulders.

Patient Data

Age: 70 years
Gender: Female

Deformity and partial destruction of the humeral head with erosion of the glenoid, marked joint space narrowing and cephalic humeral migration; in addition note the erosion of the distal end of the clavicle and acromion. Soft-tissue swelling with tiny and irregular calcifications are present.

Full-thickness tears of the supraspinatus, superior displacement of the humerus, soft-tissue swelling, large effusion and intra-articular loose body with synovial hyperplasia. The articular surface of the humeral head is extensively eroded and deformed with bone marrow edema.

Case Discussion

The differential diagnosis of this MRI appearance includes other destructive arthropathies such as Charcot (related to syringomyelia), septic arthritis, hemophilia, and amyloid arthropathy.
