Endometrial adenocarcinoma - advanced

Diagnosis almost certain

Presentation

Pelvic pain, loss of weight, and irritability.

Patient Data

Age: 35 years
Gender: Female

Uterus shows heterogeneous signal intensity measuring about 54*90*120 mm.

There is a 5* 5.6 * 8.5 cm (Vol – 120 cc) sized ill defined abnormal signal  (T1 low and T2 intermediate) non enhancing infiltrated mass lesion with marked water restriction invading the uterus body anterior and posterior walls.

The mass invades and obliterates the anterior and posterior junctional zones.

Furthermore, it appears to invade more than 50 percent of the myometrium and the outer band of the myometrium is infiltrated at left posterolateral wall and extending beyond the uterus.

Extension of the infiltrated mass into the cervix is seen.

Multiple and bilateral common iliac and internal iliac chains metastatic lymphadenopathy with maximum SAD up to 20 mm at the left internal iliac chain and 22 mm at the right internal iliac chain are seen.

No bladder / rectal involvement.

No free fluid.  

Both ovaries show the normal size and signal intensity.

Case Discussion

The patient underwent total abdominal hysterectomy and bilateral salpingo-oophorectomy, but she was unstable hemodynamically after surgery and finally expired on the 7th day after surgery. Histopathological diagnosis was compatible with Invasive endometrial adenocarcinoma.
