Presentation

Lower abdominal pain, nausea and feeling unwell. RIF tenderness with guarding, raised inflammatory markers, overweight ? acute appendicitis

Age: 30 years
Gender: Female

An area of acute inflammation of fat with a thin hyper attenuating rim close to redundant sigmoid loop is noted which is in the right iliac fossa and in the midline area.

Case Discussion

Epiploic appendagitis is a self-limiting ischemic/inflammatory process due to torsion of a large and pedunculated epiploic appendage, or spontaneous thrombosis of the venous outflow resulting in ischemia and necrosis.

Differential diagnosis include diverticulitis, appendicitis and omental infarction.
