Extrauterine migration of Mirena IUD

Presentation

Sudden onset LIF pain. Two days post Mirena IUD insertion.

Patient Data

Age: 25 years
Gender: Female
ultrasound

Uterus is anteverted. Endometrium measures 3 mm. Mirena not visible within the endometrial cavity.
Ovaries are normal.

The Mirena is located in the left adnexa between the uterus and the left ovary with the stem oriented inferiorly (see 3D Reconstructions). No free fluid.

Impression

Extrauterine Mirena IUD positioned in the left adnexa. Gynecological opinion recommended.

Case Discussion

The patient underwent laparoscopy which confirmed the Mirena had perforated through the uterine wall. The strings were still in the uterus. The Mirena was removed and the small uterine perforation diathermed. A new Mirena was inserted uneventfully.

The patient recovered well.
