Failed total hip arthroplasty

Presentation

The patient walks with steppage gait and reports feeling pain in the left thigh; the pain did not decrease despite the administration of drugs. The patient did not report any recent hip trauma.

Patient Data

Age: 65 years
Gender: Male
x-ray

Left total hip arthroplasty with stem breakage in the distal section without fracturing the femoral cortex. Diffuse periprosthetic osteolysis (bone resorption > 2 mm) with endosteal scalloping in the proximal femur probably due to particle disease is also noted involving Gruen zones 1,2,3,7,6,5. Right arthritic hip with severe joint space loss.

Case Discussion

Bone resorption >2 mm indicates local loosening and can be used as a marker for the possible need for revision. Fracture or deformation across the implant is often a sign of incomplete fixation of the implant or thinned components due to corrosion. This indicates lack of stability of the implant structure and requires revision.
