Selected images through the upper abdomen show an ill-defined, nodal soft tissue density mass lesion that is occupying pre/para-aortic, aortocaval, and renal hilar regions. The mass lesion is encasing the abdominal aorta, inferior vena cava, and renal vessels bilaterally, however, no evidence of thrombus formation is noted. No internal calcification or cystic component is appreciated. Patient has bilateral adenxal masses (not shown).

Case Discussion

Findings are in favor of enlarged and matted lymph nodes which could be either metastatic (due to adnexal lesions) or due to lymphoproliferative disorder (synchronous tumor) and displacing the abdominal aorta anteriorly more that of normal (giving the appearance of floating aorta sign) as described.
