On FLAIR FS images, there are focal thickenings of hemispherial cortex with blurred gray-white matter junction. The lesions are also accompanied by hyperintensity of subcortical white matter representing the transmantle sign. On the IR sequence, there are some hypointense bands of subcortical white matter which fan out, and focal cortical thickening. On T1W high resolution images, these lesions are hypointense.

Case Discussion

The focal cortical hyperplasia is most likely diagnosis in this case due to clinical symptoms and imaging findings.

Transmantle sign, focal thickening of cortical and blurred gray-white matter junctions are some typical features of this disease.
