Case contributed by Hidayatullah Hamidi
Diagnosis probable

Presentation

Incidental finding of well-defined opacity in chest radiograph.

Patient Data

Age: 45 years
Gender: Female

Chest

ct

A well-defined oval-shaped cystic lesion in the right side of middle and posterior mediastinum mainly above the level of carina. The lesion is abutting the vertebral bodies posteromedially, the trachea and esophagus medially and the superior vena cava anteriorly.

This homogeneous cystic lesion shows foci of wall calcification however no internal solid enhancing components or CT detectable septa are seen.

No evidence of adjacent infiltrative or inflammatory changes.

No extension of the lesion to the spinal canal.

Few calcified granulomas are seen in both lungs with calcified subcentimetric mediastinal and hilar lymph nodes.

Case Discussion

The location of the lesion and the CT features are suggestive of foregut duplication cyst (more likely bronchogenic cyst).
