Foreign body granuloma (cactus spine)

Diagnosis almost certain

Presentation

Newly developed mildly painful hard lump over the palmar aspect of the right index finger. The patient vaguely recalls a puncture injury caused by a cactus spine several years earlier.

Patient Data

Age: 50 years
Gender: Female

X-ray unremarkable. 

ultrasound

A 6 x 5 x 3 mm moderately hyperechoic nodular lesion is visible adjacent to the flexor tendon sheath. It is surrounded by a thin hypoechoic halo. No increased vascularity noted.  

Case Discussion

Organic foreign bodies such as those made of plant material often lead to formation of foreign body granulomas. In this case only a thin hypoechoic halo around the echogenic focus hints at the presence of granulation tissue.
