Gallbladder perforation with liver abscess formation

Diagnosis almost certain

Presentation

Pain in the right upper quadrant for nine days. Fever and malaise for 5 days.

Patient Data

Age: 75 years
Gender: Male
  • distended gallbladder, 45 mm in maximal diameter, with thick-walled and hyperattenuating gallstones

  • suspected gallbladder wall defects with communication tracts due to intrahepatic gallbladder perforation associated with a 30 x 40 mm abscess formation at segments V and VI of the liver abutting the gallbladder

  • 7 mm stone along with the left kidney lower calyx

  • bilateral renal hypodense cortical cysts

Case Discussion

CT imaging features are consistent with acute cholecystitis and highly concerning for perforation and hepatic abscess formation.
