Presentation

Presented with right hypochondrial colicky pain and nausea. Elevated inflammatory markers.

Patient Data

Age: 60 years
Gender: Female

Thick, enhancing gallbladder wall and cholecystopyloric fistula with a large gallstone in the gastric fundus.

Jejunal small bowel obstruction due to slightly smaller gallstone with upstream wall thickening and edematous adjacent mesentery.

Case Discussion

Operative findings correlated with the radiological findings. Jejunal resection was required due to necrosis. 

A rare case of cholecystogastric fistula due to chronic cholelithiasis resulting in gallstone ileus.
Unusual finding of a gastric gallstone.
