Presentation

Pain and swelling around the left knee.

Patient Data

Age: 25 years
Gender: Female

Knee

x-ray

A well-defined expansile eccentric lytic lesion in the lower end of the left femur involving epi-metaphyseal region with a narrow zone of transition and non-sclerotic margin. No evidence of calcification or periosteal reaction was seen. The overlying cortex appears to be thinned out.

The X-ray findings are suggestive of non-aggressive bone lesion.

Knee

A well-defined mass lesion in distal meta epiphysis of the left femur in the midline and the medial aspect. Expansion of the overlying cortex in the medial aspect with a breach in the cortex at places, however, no extraosseous soft tissue component was seen. The margin of the lesion appears well-defined with a narrow zone of transition. Minimal surrounding marrow edema. The articular cortex is intact.

The postoperative x-ray showed metallic screws and bone cement.

Case Discussion

Left distal femur GCT curettage + cement filling + internal fixation was done. The excision biopsy report showed an osteoclastic giant cell-rich tumor, consistent with a giant cell tumor of bone.

Pain in nonaggressive bone lesions can be due to pathological fracture or cortical breach.
