Giant fibroadenoma

Presentation

Left breast lump, palpable for 5 years.

Patient Data

Age: 25 years
Gender: Female
ultrasound

The left brest is almost occupied by a large heterogenous mainly hypoechoic lesion with lobulated margin showing internal cystic changes and echogenic bands measuring at least 10.8 x 5 cm.

Sono-elastography was done and shows a soft consistency.

Impression: BI-RADS IV: suspicious abnormality.

Case Discussion

*A fibroadenoma larger than 8 cm in size can be described as giant.

*Trucut biopsy was done and the pathology report is as follows:

GROSS:

Specimen fixed labeled with patient's name, consists of multiple gray white soft and fatty tissue needle core biopsies, the largest 1.9 x 0.2 cm. Submitted all in one cassette.

MICROSCOPIC:

Multiple breast core biopsies showing focal stromal fibrosis, with scattered benign ducto- lobular units. One core shows increased stromal cellularity with ducts showing intracanalicular pattern and suggestive of fibroadenoma. Mild nuclear atypia is noted in some stromal cells, but no mitotic activity seen. Some ducts show apocrine changes. No malignancy seen in the levels examined.

DIAGNOSIS:

Benign breast tissue, with features suggestive of complex fibroadenoma.
