Glaucoma with optic atrophy

Age: 70 years
Gender: Male

Both globes are enlarged axially (right 27.5 mm, left 25.8 mm). The optic nerves are small, particularly on the right with commensurately small right side of the chiasm. The right optic nerve also has slightly increased T2 signal. No other intra or extraconal abnormality. The extraocular muscles, the intraorbital fat, and lacrimal glands appear unremarkable. The remainder of the brain is unremarkable with no intra- or extra-axial abnormality.

Conclusion: Increased axial length of both globes, more so on the right, with optic atrophy is attributable to known severe glaucoma. No other cause identified.

Case Discussion

This man had known severe glaucoma (diagnosed by ophthalmology) with optic nerve neuropathy. The MRI was request to ensure there was no other pathology (e.g. a compressive lesion).
