Presentation

GCS 3

Patient Data

Age: 30 years
Gender: Male

Diffuse cerebral edema is noted with loss of gray white matter differentiation and effacement of the sulci and cisterns.

No acute hemorrhage or space occupying lesion. No mass effect or herniation or midline shift.

No hydrocephalus or extra-axial fluid or hemorrhagic collections.

Case Discussion

The patient presented with unwitnessed seizure or fall and arrested for 2 mins with pulseless electrical activity, return of spontaneous circulation following resuscitation. Clinical examination revealed dilated and not reactive pupils with GCS 3.

Severe global hypoxic-ischemic injury in adults show variable pattern of involvement. Most often it involves the cortex, fairly diffusely, along with the deep grey matter structures, particularly basal ganglia.
