Graves disease

Diagnosis almost certain

Presentation

Patient presented with weight loss, generalized fatigue and diarrhea. Bloods showed low TSH with raised T3 and T4.

Age: 45 years

Tc-99m pertechnetate scan

Nuclear medicine

The thyroid gland is normal in size and entirely suprasternal (compared to the SSN [suprasternal notch] marker).

There is homogeneous uptake within both lobes of the thyroid gland. This uptake is homogenously increased when compared to the normal salivary glands.

The patient has a neck-to-thigh ratio of 17.6, which quantitatively supports the finding of increased uptake.

Case Discussion

Given the clinical history of hyperthyroidism, the Tc-99m pertechnetate thyroid scan findings are consistent with Graves' disease.

Following this scan, the patient was diagnosed with Graves' disease and treated with carbimazole. At the 3-month follow-up with the endocrinologist, the patient reported improved symptoms and showed improvement in TSH.
