Diagnosis almost certain

Presentation

Right breast painful condition with mild enlargement and tenderness for 6 months.

Patient Data

Age: 45 years
Gender: Male
ultrasound

Right breast retroareolar triangular/pyramidal-shaped hypoechoic mass with dendritic margins and apex abuts the deep surface of the nipple. It shows internal vascularity.

Left breast is normal, no retroareolar masses.

Case Discussion

Gynecomastia is the most common male breast abnormality. Unilateral presentation is more commonly than bilateral. The characteristic retroareolar location and pyramidal shape differentiate it from malignancy. Biopsy is not indicated. It is considered a Birads II lesion recommended for clinical follow-up.
