Haemophilic arthropathy

Diagnosis almost certain

Presentation

Right knee twisted since 2 years, currently complains of contracture and muscle atrophy. He has chronic diseases.

Patient Data

Age: 10 years
Gender: Male
x-ray

Marked right knee joint degenerative changes, as

  • periarticular osteopenia and subchondral erosions

  • epiphyseal enlargement and the physis is narrow compared to the left knee

  • markedly narrowed patellofemoral and tibiofemoral joint compartments

  • widened intercondylar notch

  • evident irregularities of articular surfaces of tibial and femoral epiphyses

  • joint disorganization

There is resultant right lower limb shortening.

The left knee shows mild diffuse narrowing of tibiofemoral joint spaces with subchondral cystic changes.

Case Discussion

A 10-year-old patient known to have diabetes mellitus and hemophilia. The knee joint is the most common site of haemophilic arthropathy with more predilection to the right side. It is considered right stage V and left stage III according to Arnold-Hilgartner classification.

The main differential diagnosis is juvenile rheumatoid arthritis.
