Presentation

Primary amenorrhea and abdominal pain.

Patient Data

Age: 12 years
Gender: Female
mri

Marked distension of the vagina with blood signal being of bright signal on T1 and T1 fat sat sequences and intermediate T2 signal.

Normal appearance of the uterus and the endometrial cavity.

Normal appearance of both ovaries.

Case Discussion

Hematocolpos is defined as accumulation of menstrual blood in the vagina usually due an imperforate hymen leading to primary amenorrhea in adolescent girls.

Other caused include: transverse vaginal septum, vaginal atresia and hemi vaginal atresia.

Clinical examination can reveal imperforate hymen, US can be misleading as it can be confused with cystic adnexal lesions, MRI usually confirms the diagnosis.
