Patient Data
duplicated uterine bodies and cervices open in separate vaginal cavities (uterine didelphys)
the right vagina is obstructed and distended by a high signal in T1 and intermediate T2 with no signal drop in T1 fat sat denoting hemorrhagic content (haematocolpus)
a similar signal yet to a much lesser extent is seen at the right uterine horn
the left vaginal cavity is collapsed and seen stretched over the distended right vagina
Case Discussion
This is a classic case of Herlyn-Werner-Wunderlich syndrome (also known as OHVIRA syndrome) which is characterized by a triad of type III Müllerian duct anomaly (uterus didelphys), obstructed hemivagina, and mesonephric duct anomalies as renal agenesis in this case.
