Left high frontoparietal parafalcine subcortical ill defined space-occupying lesion is eliciting low signal on T1 WI and high signal on T2 and FLAIR with small patchy areas of diffusion restriction and small punctate post-contrast enhancement. The lesion extends for a short distance along the left corticospinal tract and also infiltrates the splenium of the corpus callosum. It exerts a positive mass effect in the form of effacement of the overlying cortical sulci. MRS showed elevated choline peak and Cho/CR ratio and reduced NAA peak.

Radiological findings are suggestive of left frontoparietal high-grade glioma.

Pathology after excision showed high-grade glioma.
