HIV-associated varicella-zoster myelitis

Presentation

T10 left-sided segmental pain with subacute ascending Brown-Séquard syndrome. Herpes zoster vesicular rash in the left T10 dermatome.

Patient Data

Age: 45 years
Gender: Male

T2 images of the spinal cord demonstrate a hyperintensive lesion from the T10 to T11 level with associated mild enhancement.

Case Discussion

The patient has been immunocompromised and untreated HIV positivity for 10 years.

СBC: lymphocytes 0.7 * 109 /L (normal range 1-4.8).

Serum analysis revealed the presence of cytomegalovirus and varicella-zoster virus.

CSF analysis was positive for the varicella-zoster virus.

A diagnosis of HIV-associated varicella-zoster myelitis was therefore made.
