Hypothalamic cavernous venous malformation

Case contributed by Nick Tarasov
Diagnosis certain

Presentation

Follow-up for a known brain mass in an asymptomatic patient.

Patient Data

Age: 45 years
Gender: Female

There is a midline lobulated mass up to 24 mm within the hypothalamus with heterogeneous high T2 signal and peripheral hemosiderin deposition.

Typical popcorn appearance on the gradient echo sequence.

Non-specific white matter foci within frontal lobes.

Remainder of brain looks unremarkable.

Case Discussion

The patient had an unexplained episode of anemia 10 years ago, brain MRI revealed a hypothalamic mass with hemosiderin deposition, stable in time.

Despite the mass' location in the suprasellar region, the patient is asymptomatic.

This is a rare location for a cavernous venous malformation; the differential diagnosis of craniopharyngioma was excluded due to stability in size mass.

Further follow-up only, since the mass is asymptomatic.
