Presentation

Sudden left hip and pelvic pain onset during an athletic event (sprinting ). Request for plain films of the hips.

Patient Data

Age: 15 years
Gender: Male

Apophyseal avulsion fracture of the left iliac crest is present.

The bony pelvis and especially the hips are otherwise intact and normal

A zoomed and panned image confirms the left iliac crest apophyseal avulsion fracture injury.

Case Discussion

The patient was referred for a dedicated hip X-ray series including frog leg and Dunn views. The iliac crest apophyseal avulsion fracture cannot be diagnosed without a pelvic view! This can also be easily missed when the views are collimated during exposure or cropped during post-processing while concentrating on the hip request.

Apophyseal avulsion fractures of the pelvis and hip are relatively common among physically active adolescents and young adults.
