Abnormal sclerotic line of impaction is seen in the right medial femoral neck with a minimal medial rotation of the femoral head giving a“mushroom cap "appearance. There is valgus impacted right neck of femur fracture.
Case Discussion
Impacted femoral neck fractures are also referred to as valgus impacted fractures and are classified as Garden stage 1 fractures.
In elderly patients, the Garden classification of sub-capital neck fractures is usually used:
Stage 1: incomplete or valgus impacted.
Stage 2: complete without displacement.
Stage 3: complete with partial displacement.
Stage 4: complete and totally displaced.
These fractures can be managed surgically with cannulated screws or arthroplasty based on the functional status of the patient. Some cases can be managed be conservative based on patient physiology.
