Impaction fracture and bone bruise of the proximal tibia

Diagnosis probable

Presentation

Fall whilst running upstairs, hitting anterior leg with immediate pain.

Patient Data

Age: 30 years
Gender: Female

Focal region of edema within the proximal tibia centered on and surrounding the insertion of the iliotibial band (ITB), with associated minimal impaction of the anterior cortex of the tibia. Edema surrounds the distal ITB with mild expansion and mild disorganization of its fibers. Edema extends into the overlying subcutaneous tissues and into the proximal muscle belly of tibialis anterior.  

Intact cruciates, collaterals and meniscus. Mild fraying of the patella articular cartilage. 

Findings in keeping with a minimally impacted anterolateral tibia fracture, associated bone and soft tissue bruising and partial thickness tearing and contusion of the distal ITB. 

Case Discussion

Bone bruising is the preferred term for focal marrow edema change instead of terms such as trabecular microfracture 1.  The placement of oil bead markers on the region of concern can help focus interpretation.
