Incidental right terminal ICA aneurysm on CT sinuses

Diagnosis almost certain

Presentation

Persisting headache. History of chronic sinusitis.

Patient Data

Age: 70 years
Gender: Female
  • focal saccular dilatation of the terminal right internal carotid artery with peripheral calcification, suspicious for an incidental ICA aneurysm
  • the paranasal sinuses are clear
  • the CTA confirms that there are in fact two supracavernous right ICA aneurysms measuring up to 9 mm
  • both aneurysm sacs project superiorly and to the right

Case Discussion

This patient presented with headache and illustrates that although this is a CT sinuses study that the soft tissue and vascular structures remain an important check area.

The patient went on to have a dedicated CTA, which confirmed the aneurysms. The patient was  then referred to a tertiary center for neurosurgical and neurointerventional input.
