Presentation

Evaluation of a chronic diarrhea

Patient Data

Age: 50 years
Gender: Female

There is an abnormal position of the duodenojejunal (D-J) flexure which is located to the right of the midline (normally to the left), posterior to the mesenteric vessels while the third duodenum is anterior to the cited vessels. The cecum is in the midline in a transverse position, anterior to the third duodenum and in contact with the anterior abdominal wall. The SMV  is dilated, revolves around the superior mesenteric artery giving the appearance the "Whirlpool sign"(mesentery). Note dilated jejunal and ileal veins (branches of the SMV).

Case Discussion

CT features are consistent with an incidental incomplete intestinal malrotation (180 degrees)
