Presentation

Confusion and headaches

Patient Data

Age: 80 years
Gender: Female

Asymmetric confluent edema involving the right parietal, temporal, and occipital lobes, where multiple microbleeds are seen on the T2* FFE sequence, and leptomeningeal enhancement on post-contrast sequences. Moreover, there is effacement of cortical sulci and mass effect on the right lateral ventricle.

Another area of edema with microbleeds and subtle leptomeningeal enhancement on the right frontal lobe.

Superficial hemosiderosis in the left occipital lobe and bi-hemispheric scattered microbleeds are seen.

Case Discussion

A probable case of inflammatory cerebral amyloid angiopathy with asymmetric multilobar edema involving the subcortical white matter and multiple microbleeds.
