Insufficiency fractures due to vitamin D deficiency

Presentation

Pain and limping in bilateral lower limbs. No history of trauma.

Patient Data

Age: 15 years
Gender: Female

X-ray Pelvis (AP view)

x-ray

Linear lucencies in the bilateral femoral neck with cortical discontinuity.

MRI both hip joints

Linear hypo intensity in the bilateral femoral neck on T1W/T2W images with mild surrounding bone marrow edema.

Follow up X-ray (hip joints)

Follow-up X-ray after two months showing near complete healing of bilateral femoral neck insufficiency fractures.

Case Discussion

The imaging findings and low serum 25-OH Vitamin D level ( 9.5 ng/ml ) are suggestive of insufficiency fractures due to vitamin D deficiency.

After treatment with therapeutic dose of vitamin D, Follow up X-ray after two months shows near complete healing of bilateral femoral neck insufficiency fractures with normal serum 25-OH Vitamin D level ( 32 ng/ml ) .
