Presentation

Syncope.

Patient Data

Age: 50 years
Gender: Female

CT head normal, except for fullness in left cavernous sinus, with bone remodeling of sphenoid sinus and truncation of left anterior clinoid process - suspected internal carotid artery aneurysm.

The patient was returned for a CT angioraphy.

Saccular aneurysm 10 mm in maximal diameter and 18 mm in length, arising from the C4 (cavenous) segment of the left internal carotid artery.

Case Discussion

Luckily, the radiologist identified a bulge in the left cavernous sinus on non-contrast CT and raised the suspicion of an aneurysm, which was verified on CTA.
