Case contributed by Masoud Farhadi
Diagnosis almost certain

Presentation

Abdominal pain.

Patient Data

Age: 30 years
Gender: Female
ultrasound

Transvaginal ultrasound images of the uterus demonstrate a cystic, rounded structure with an echogenic peripheral rim in the region of the uterine cornu.

This structure refers to the gestational sac because of the positive BHCG, empty endometrial cavity, decidual reaction, and no adnexal lesion on either side.

An echogenic line extends from the endometrium to the gestational sac (interstitial line sign).

The myometrial thickness around the gestational sac is 2.5 mm.

The mean sac diameter is 8 mm.

No fetal pole and no yolk sac are seen.

Case Discussion

This case demonstrates an interstitial ectopic pregnancy based on the following findings:

  • positive BHCG

  • empty endometrial cavity

  • myometrial thickness around the gestational sac < 5 mm

  • interstitial line sign

  • no adnexal lesion on either side.
