Presentation

Headache and seizure.

Patient Data

Age: 60 years
Gender: Male
mri

Irregular T1WI hypointense and T2WI hyperintense cystic mass lesion is seen on the right side of the brain related to the temporal and ganglionic-capsular regions and the thalamus with associated mass effect. It is seen extending medially into the right crural and the chiasmatic cisterns. Effacement of sulci, compression of the 3rd ventricle & temporal horn of the right lateral ventricle, and contralateral midline shift are seen. The lesion shows incomplete attenuation in FLAIR, intense diffusion restriction, and negative enhancement in the post-contrast study.

Case Discussion

Incomplete FLAIR attenuation and Intense diffusion restriction are typical MR features of the intracranial epidermoid cyst.
