Presentation

An incidental finding in a trauma patient.

Patient Data

Age: 60 years
Gender: Male
ct

"A well-defined narrow zone of transition lytic lesion is seen in the right 6th rib posteriorly with fat density suggesting an intraosseous lipoma.

Incomplete fractures at the anterior aspect of the left 5th, 6th, 8th, and 9th ribs proximal to costochondral junctions.

A minimally displaced fracture is noted at the anterior aspect of the left 7th rib.

The heart size is enlarged.

The pulmonary trunk is dilated, measuring about 3.7 cm, indicating pulmonary hypertension.

Multiple gallbladder stones are noted.

Case Discussion

Intraosseous lipomas are uncommon incidental findings; however, the majority of cases present with pain due to stretching of the periosteum, which may resemble the pain of osteoid osteoma.
