Isolated medial malleolar fracture

Presentation

Pillion rider who fell after a road traffic accident resulting in right leg being pinned down by motorcycle. Currently unable to weightbear, with right ankle abrasion observed.

Patient Data

Age: 40 years
Gender: Female

Ankle X-ray: There is a minimally displaced transverse fracture of the medial malleolus with no dislocation observed. Associated soft tissue swelling is seen at the ankle.

Knee X-ray: There is no acute fracture nor dislocation noted, and no presence of suprapatellar effusion as well.

Case Discussion

Isolated medial malleolar fractures occur in approximately 1 out of 5 ankle fractures with some even concluding that such fractures may have a worse prognosis compared to isolated fractures of the lateral malleolus 1. Additionally, for medial malleolar fractures with no accompanying fibular fracture (i.e. Maisonneuve fracture), they are classified as partial articular distal tibial end segment fractures 2.
