Presentation

Jaundice and weight loss.

Patient Data

Age: 65 years
Gender: Male

There is marked dilatation of the left hepatic duct, with a faint enhancing slightly hyperdense mass within the common hepatic duct at the porta hepatic region with extension into the left duct associated with changes at the same lobe.

The pancreas and pancreatic duct seem normal. 

No significant para-aortic lymphadenopathy.

No ascites.

The gallbladder, spleen, pancreas, both kidneys, and adrenals are unremarkable.

Case Discussion

Imaging features are highly suggestive of an intraductal-growing hilar cholangiocarcinoma (Klatskin tumor) at the bifurcation of the common hepatic duct with extension into the left hepatic duct. It is categorized using Bismuth classification stage IIIb.
