L2/L3 disc extrusion with neural impingement

Diagnosis certain

Presentation

Acute onset of lower back pain; pain in the back and down right leg. Likely prolapsed disc L3/L4. Altered sensation. No retention. SLR limited to 60 degrees.

Patient Data

Age: 75 years
Gender: Male

Large L2/L3 right paracentral disc extrusion with cranial extent of the disc almost to the L1/L2 disc space. The extruded disc fills the right lateral recess and impinges on the descending L3 nerve root as well as impinging on the exiting L2 nerve root at the foraminal origin.

Minor diffuse disc bulge, facet joint degenerative changes and ligament flavum hypertrophy at L4/L5 causing minor central canal stenosis.

T12 conus. Normal cauda equina.
