Diagnosis probable

Presentation

Lower back pain and left-sided radicular symptoms. Spontaneous onset, no previous injury or trauma.

Patient Data

Age: 60 years
Gender: Female

L3 vertebral body lesion

An extradural lesion posterior to the L3 vertebral body, within the left lateral recess. Indentation present at the anterior thecal sac on L3 level with resultant pressure effect on the traversing left L4 and exiting L3 spinal nerve roots.

Case Discussion

Disc sequestration refers to the extrusion of disc material away from the parent disc.

Patients commonly present with radiculopathy symptoms such as pain, weakness, and paresthesia affecting the dermatome of the injured disc. They may occur due to trauma, age, poor posture, and genetics.

MRI is the gold standard imaging modality used in diagnosis. The toothpaste sign is a typical finding.

Differentials include metastasis, discal cyst, neuroma, and epidural masses such as hematomas or abscesses.
