Patient Data

Triple phase CECT abdomen

ct

A large, well-defined, lobulated, heterodense lesion arising from the left lobe of the liver with no enhancement on post-contrast images. Multiple small, hyperdense areas are seen within the lesion, likely representing hemorrhageMild IHBR dilatation in the left lobe of the liver, likely due to mass effect. Mild ascites are seen.

Case Discussion

The CBC of the patient showed decreased hemoglobin (7.6 g/dl) with neutrophilic leukocytosis (20.16 x 103/microL). The amoebic serology (IgG) was positive. The ultrasound-guided pigtail drainage was done and showed hemorrhagic aspirate at one site and anchovy sauce-colored aspirate from another site of the lesion.

Overall findings suggest a large amoebic liver abscess with intralesional hemorrhage.
