Large bowel obstruction due to abdominal wall hernia

A 50 mm midline fascial defect is present in the anterior abdominal wall, and some omental fat and a part of the transverse colon are herniated through it, causing obstruction and proximal bowel loops dilatation. There is no obvious sign in favor of colon wall ischemia in the obstruction site.
The right kidney is markedly small and shows severe parenchymal atrophy without stone or hydroureteronephrosis.
