Case contributed by Stefan Tigges
Diagnosis almost certain

Presentation

Abdominal distension.

Patient Data

Age: 60 years
Gender: Male

Multiple distended loops of small bowel containing air-fluid levels with dilated ascending colon with normal caliber transverse and distal colon, suspicious for proximal large bowel obstruction.

Enhancing colonic mass narrowing the distal ascending colon just proximal to the hepatic flexure, likely a primary colon cancer. Resultant bowel obstruction proximal to the lesion with fluid and gaseous distention of the cecum. No definitive findings of metastatic disease. Cholelithiasis, prostatomegaly.

Case Discussion

The most common cause of a large bowel obstruction is colon cancer.
