Large skull metastases: primary follicular thyroid carcinoma

Presentation

A large occipital mass lesion with known background thyroid follicular carcinoma. For CT assessment prior to incisional biopsy.

Patient Data

Age: 60 years
Gender: Female

There is a large, heterogeneous, enhancing, occipital soft tissue mass, with regional bone erosion and intracranial extension. There are scattered peripheral/ rim calcifications. The mass measures 11.5 x 9.8 x 6.7 cm ( cc x w x AP).

Prominent superficial draining veins.

Contrast-enhanced CT brain is otherwise normal.

Features consistent with metastatic follicular carcinoma of the thyroid gland on incision core biopsy of the occipital mass.

Case Discussion

An example of a histopathologically proven metastatic follicular thyroid carcinoma. The status of the primary malignancy and previous management thereof was unknown at the time of presentation and CT imaging.
