Lateral patellar dislocation with large osteochondral fragment

Presentation

ACL rupture?

Patient Data

Age: 25 years
Gender: Female

Large lipohemarthrosis. 20 mm intra-articular osteochondral fragment adjacent to the lateral femoral condyle.

No meniscal injury. Collateral and cruciate ligaments are intact.

Quadriceps and patellar tendons are intact.

Lateral translocation of the patella without subluxation. Femoral trochlear appears well-formed. Small bone bruise at the inferomedial patella. Chondral defect of the central patella. MPFL insertion high signal, although no redundancy to suggest a full-thickness tear.

Mild myofascial edema at the inferior aspects of both vastus medialis and lateralis.

Mildly depressed cortical impaction fracture at lateral femoral condyle with minor chondral injury at the periphery of the lateral femoral trochlea.

Case Discussion

This is a case of transient lateral dislocation of the patella, now relocated, with large hemarthrosis and a displaced osteochondral fragment.
