Age: 25 years
Gender: Female

Fracture is seen in both plates of the right pterygoid bone. Fractures are seen in the frontal process of the right maxilla, anterior and postero-lateral walls of the right maxillary sinus, right zygomatic arch, and fronto-zygomatic suture.

Fractures are seen in the postero-lateral and inferior walls of the right orbit. Right orbital fat is herniated into the right maxillary sinus. The inferior rectus muscle is normal in position. Fracture is seen in the left medial pterygoid plate, frontal process of the left maxilla, infraorbital part of the left maxilla, and body of the zygomatic bone.

Right-sided proptosis and pre-orbital soft tissue swelling are seen. Air is seen within right orbit, pre-orbital regions, and masticator space.

The hemorrhagic collection is seen in both maxillary and ethmoid sinuses.

Case Discussion

Involvement of right pterygoid plate, frontal process of right maxilla and zygomatic arch suggest right sided type III Le Fort fracture.

Involvement of left pterygoid plate, frontal process of left maxilla and infra orbital part of maxilla suggest left sided type II Le Fort fracture.

In addition right sided orbital blow out fracture is also evident with herniation of orbital fat into right maxillary sinus.
