Left atrial enlargement

Diagnosis almost certain

Presentation

Shortness of breath.

Patient Data

Age: 40 years
Gender: Female
x-ray

Normal cardiothoracic ratio (0.48). Prominent left atrial appendage (third mogul sign) and double density sign along the right heart border. On the lateral, the atrial silhouette is superimposed over the vertebral column. 

No pleural effusion or pulmonary edema. 

Case Discussion

A normal cardiothoracic ratio does not mean a normal heart. The heart subjectively looks "big" in this case even though the objective measurements are normal. Interrogation of the image beyond the basics reveals signs of left atrial enlargement, which had been confirmed previously on echocardiography along with moderate/severe mitral stenosis and mild/moderate aortic regurgitation.
