The Lichtman classification of Kienböck disease is divided into:

  • stage I: normal radiograph

  • stage II: lunate sclerosis

  • stage III

    • IIIA: lunate collapse

    • IIIB: lunate collapse and carpal instability

    • IIIC: chronic coronal lunate fracture

  • stage IV: carpal collapse and perilunate arthrosis (Kienböck disease advanced collapse)
