Lithium-induced nephrotoxicity

Case contributed by Michael P Hartung
Diagnosis almost certain

Presentation

Left lower quadrant pain.

Patient Data

Age: 50 years
Gender: Male

Normal-sized kidneys with diffuse cortical microcysts. Left upper pole renal mass. Descending colitis.

Case Discussion

There’s a lot to look at here! Aside from the typical findings of lithium kidneys, there is also a suspected left RCC which should undergo biopsy. The pattern of descending colitis is typical of nonocclusive ischemic colitis and the reason for the patient's presentation.
